The University of Kansas Panhellenic Association (PHA) is committed to promoting the health and safety of students on campus. This page is intended to connect you to resources that will secure your wellness at KU and within Sorority and Fraternity Life.
The University of Kansas Panhellenic Association has a longstanding partnership with the University’s Office of Student Conduct and Community Standards. This partnership allows our organization to investigate serious incidents of non-academic misconduct and violations of the student code, move to resolve them, and protect the dignity and confidentiality of the parties involved.
Due to the organizational complexities faced by Panhellenic member chapters, we encourage PHA members to resolve minor incidents within their own chapter or through Panhellenic mediation. If you would like more information about PHA judicial procedure, mediation, and filing infractions, click here.

When do I use this form?
This is to report alleged acts of hazing at KU.
According to studentconduct.ku.edu: Instances of hazing are including but not limited to...
Endangering one’s mental health or mental discomfort which includes, but is not limited to:
Yelling
Demeaning names
Lineups
Kidnapping
Blindfolds
Requiring embarrassing attire or activities involving nakedness
Embarrassing activities (scavenger hunts)
Endangering one’s physical health or physical discomfort which includes, but is not limited to:
Tests of endurance
Consumption of alcohol or non-food substances, large amounts of water or other liquids
Exposing members to extreme weather conditions
Sleep deprivation (early morning wake-ups, not allowing naps, specific bed times)
“Fountaining” (as admission to or for membership in a group, students are ‘placed’ in the fountain)
Personal servitude includes, but is not limited to:
Cleaning an active member’s room
Repeatedly running errands for an active member; including but not limited to, “wake-ups,” breakfast, pick-ups, and drop-offs.

When do I use this form?
This is where you would go to report Title IX violations and allegations of discrimination and harassment.
Information from IOA Website: “IOA is a neutral fact finder conducting investigations in response to allegations of discrimination and harassment. IOA will investigate allegations of discrimination and harassment complaints when the complainant believes the behavior is based on race, color, ethnicity, religion, sex (including sexual harassment and sexual violence), national origin, age, ancestry, disability, status as a veteran, sexual orientation, marital status, parental status, retaliation, gender identity, gender expression and genetic information.”
*This office recently went under a rebrand and was previously known as the IOA (Office of Institutional Opportunity and Access). This office operates the same way and all the reporting procedures remain the same.*

When do I use this?
To report instances of egregious violations of the Protect KU Policy.
Things to know when reporting…
Provide as much detail as possible about who (individual/organizations) was involved
Include as much detail possible about what/where/when the incident occurs
Include documentation (photos, videos, screenshots, and anything else) to assist in the process
Include your information if you feel comfortable. (It is hard to follow up with an investigation if there is no contact information provided)
Hotspot Mapping Policy: If you are the survivor of an incident of non-academic misconduct and would like to report anonymously, that is an option through the Office of Civil Rights and Title IX reporting form. However, the investigative capabilities of the office will be significantly more limited. The office is capable of using hotspot mapping technology to identify hotspots of reports geographically. If a cluster of reports in the same area are identified, that gives the office the ability to take preventative measures to target the culture of that specific location, and implement some strategies to reduce risk. However, this would not be directly tied to an individual or a specific alleged incident, and would instead be reflective of more pervasive issues within a given location.
PLEASE NOTE: According to the Manual of Information set forth by the National Panhellenic Conference,
“NPC sororities are united in our obligation to chapter leaders, volunteer advisors and organization staff and cannot support any attempts to designate them as mandatory reporters under Title IX or Campus Security Authorities under the Clery Act.”
PHA Executive Board members are not mandatory reporters and thus have limited investigatory and reporting capacities. For more information on mandatory reporting responsibilities and limitations, click here.

KU CARE Services
Free, confidential, and voluntary support on-campus for students who have experienced interpersonal violence (including sexual assault, abuse, & harassment; stalking; and relationship/dating violence). Care is student-centered, available to all genders, and focused on supporting someone’s decisions about what’s best for them. Someone does not have to talk about their experience or share any details to access support.
Services might include emotional support, academic support, personal advocacy, and information about resources and options both on- and off-campus. Can provide referrals to post-harm medical services, mental health care, legal resources, and reporting processes.

Lawrence Memorial Hospital
Provides confidential medical care. Also provides Sexual Assault Nurse Exams (SANE) 24/7. The responsibility of a SANE includes collecting and documenting any forensic evidence. Survivors of sexual assault should be seen immediately (no greater than 120 hours). SANE is free and confidential.
Note: A Sexual Assault Nurse Examiner (SANE) is a registered nurse who has received specialized training in forensic evidence collection and medical care of sexual assault victims. Along with the forensic exam, the SANE will provide medical care such as preventive medication for common sexually transmitted infections, disperse emergency contraception, as well as treat and document injuries.
